The Lord's supper is a remembrance and outward sign reflecting an inward reality of the believer, like a profession of faith and the baptism of repentance.
The believer has sipped from the cup of mercy and is cleansed by the blood of Christ; is fed by the bread of His grace, partaking in the body of Christ.
It is a symbol and means of grace, but is not the grace alone or of itself.
We take communion each time we commune with God.
